    I wanted to ask if anyone would be willing to share suggestions or templates for Chaplain documentation?

    Good morning Melissa,

    This is something we're trying to improve on ourselves.  So we are starting to ask our Chaplains to specifically state when they are providing bereavement support as this seems to be important for demonstrating to auditors that bereavement support was provided to both the patient and their circle of care throughout their time on service.  Interventions that we are often seeing them document are things like: ministry of presence, prayer, validation of emotions, reflective listening, education about pre- and post-death bereavement, referrals to outside organizations, etc. I'd be interested to see what language others are using as well.

    We provide a complimentary template to our customers, but I am happy to share it with you and anyone else who might need something like that.

    You can find it on our website https://www.hopethroughhealing.com/self-customization/ under the "GMS Tracking form". It's intended to be used with our Grief Mail Series, but might work for your purposes.
