We are using the bereavement assessment both pre and post-death as a means of measuring the effectiveness of our overall program as we believe that our program needs to show a clear progression. The assessment is one of the few ways we can do that other than the bereaved person or the patient deciding that they are in a good enough place to terminate support. My suggestion is to review the federal and state regulations to determine what aspects of the assessment are most important for measuring. But for the patient specifically the minimum is usually anxiety and depression severity and frequency ratings.
