  • 1.  Bereavement Care Plan Based on Risk Level

    Hello!  We're evaluating our bereavement program and I'd like to get a sense of how other hospices individualize the bereavement care plan based on the assessed bereavement risk level.  For example, what is your baseline bereavement contacts for all bereaved?  And then how do the contacts/interventions change based on a medium or high risk level?

    I would say that level of risk is just one factor to consider in determining frequency of follow up contacts. Some low risk bereaved clients ask for continued check in calls. Our frequency for routine low risk follow up calls is quarterly contacts. Other low risk bereaved may only want mailings. Some moderate to high risk individuals may request a visit with a counselor and could benefit from several subsequent visits at intervals decided on together. Other moderate or high risk individuals may have complicated factors that warrant referring them to other community professionals for ongoing therapy. Our bereavement counselors do not provide that, so even though the individual is high risk, they may only get mailings from us, because they were referred elsewhere for more intensive support.

    Hi Katy, our baseline is to contact each person to provide a bereavement assessment. When we initiate the first call depends on the level of risk and then subsequent calls are based on the individuals assessment. Our higher risks we try to call within a couple weeks and gradually increases with risk level. Everyone receives mailings unless they decline them. Further follow up is individualized and based on assessed need.

    Our guideline is to contact every bereaved by phone to assess their needs, offer mailings, limited individual support, and information about our virtual grief support group meetings. Depending on the assessment, bereavement risk level, and the openness to support, calls may be made by a volunteer for low risk or bereavement coordinator for medium or high risk. We may refer to outside resources for therapy, in-person groups, or groups tailored to their specific needs -- children's grief, for example.

    We just revised/finalized our Consolo templates for "routine" bereavement care plan interventions for Low, Moderate, and High Risk assessments and we went with the following schedules:

    LOW

    Initial Bereavement Phone Call (Chaplain) - 2 days

    1st Letter w Grief education materials (Bereavement Coord/volunteer/staff) - 3 weeks

    2nd Letter w local/online support group options - 3 months

    3rd Letter w Grief Self-Assessment tool - 6 months

    Phone call at/around Birthday of Pt (Chaplain or Bereavement staff) - Sometime during the week preceding birthdate

    Anniversary of Death phone call w discussion anticipating 4th and final routine care letter coming within the next month- Sometime during the week preceding the 1 year mark since death of Pt.

    Final letter advising end of routine care contact w remainder bereavement support remains avialable upon request in perpetuity - 13 month

    MODERATE

    Added to the above are phone calls from chaplain or bereavement staff at the 30 day mark to see if risk assessment is proving to be on target or not. Assuming so, another phone call at 90 days to check-in and highlight list of bereavement support groups being included in 3 months Letter 2, now that bereaved's grief has had time to settle into whatever state it will likely maintain and presence of enduring complicated grief can be more confidently determined.

    HIGH

    Added to the above are an additional phone call check-in at 60 days and then again at 6 months, particularly to inquire as to how well any grief support group participation, referred professional grief counseling/therapy, or other intervention efforts are coming along in the bereaved's experience to-date.

    Additionally, in all letters and any phone calls we include a reminder and encouragement to save the date regarding the next occuring Memorial Gathering event.
