I am trying to gain more clarity about the expectation around hospices providing bereavement support in nursing homes (standard 418.112c). How is your hospice doing this and how are you documenting those efforts within your electronic medical record? Our hospice provides a lot of support to nursing home residents in the form of support groups, worship services, memorial services. Are we expected to be providing one one on one support to individuals (not patients on our hospice), adding them into our EMR and charting those efforts? Any insights appreciated. Thanks!
