  • 1.  Bereavement Policies

    Hello.  We are in the process of trying to update our current bereavement policy and procedures, and hopefully align them with the latest NHPCO Guidelines for Bereavement Care.  Does anyone have, or know of,  a fairly general Bereavement Policy and Procedures they could share that we might use as a guideline? Thank you so much!

    Dear colleagues,

    Yes, must agencies will not share their guidelines for many reasons but I personally believe that you must develop guidelines according to your context. For example: It is not the same to serve the population of Los Angeles that to serve the population in Orange county. They may be only 30 miles away but they sure are different and thus, unique.  I have worked in a few agencies as bereavement coordinator since 2015 and I have to say that all of them follow Medicare guidance and mandates but are contextualize. Based on that then, I am sure you will be able to benefit by reading the 2017 or 2018 GUIDELINES FOR BEREAVEMENT CARE IN HOSPICE by National Hospice and Palliative Care Org.  

    Another factor to consider is what software your agency uses. Home Care Home Base and Hospice MD have pre-defined bereavement POCs focusing on High, Moderate and Low risk. You can very much follow that and integrate things based on the context needs to define things like: Bereavement assessment at admission, during decline and post-mortem follow up of 13 months which is in the software you use, include your annual memorial celebration, bereavement follow up phone calls/ visits (PRN), quarterly grief and grieving letters and newsletter,  grief support groups and other community resources. Again, follow the frequency of the POC that your software has and you have a solid bereavement program.

    Hi there
    Our bereavement Policy essentially mirrors the Medicare conditions of participation. Then we have standards that outline what our hospice requires to be a quality and compliant bereavement program. I think some hospices would include things like that in their policies. So things like providing community bereavement care, bereavement mails bags, support group offerings etc. we als have a standard that spells out when we the thebbereavement assessment as part of the initial comprehensive assessment, how we assess post death, care plan development etc. 
    hope that helps. You might get other ideas from the NHPCO Bereavemebt guidelines.
