  • 1.  Capturing Bereavement Participation in IDG

    Hello -

    I have a question for those of you who are bereavement coordinators who attend all of IDG.  We start our meeting discussing deaths which I go over most recent bereavement risk assessment, if everyone is in agreement and if there is anything pertinent I need to know outside of Pt's chart.

    I also attend the entire time so I can take notes on Pts and hear more about them and their family to have a better understanding of needs and family dynamics prior to death and so we can identify who need pre-bereavement care as well. 

    My question is - for those of you who use HCHB how do you capture your participation in IDG (reviewing existing bereavement risk assessment/offering suggestions/ etc) in HCHB.  Do you do it in the IDG console?  Or in the bereaved' s chart?   Looking for ways to capture that we are reassessing and discussing bereavement during IDG for most patients.

    Hello - we use HCHB and also attend every IDG in it's entirety.  We put our notes in the IDG console for both the death and start of care, and also if there's something pertinent to bereavement during the patient's stay.  We utilize the "Plan of Care Update" from the death IDG so we can see the notes from all the disciplines before contacting the bereaved for the first time.   Hope this is helpful!

    We have been instructed that if there is no bereavement contact/visit prior to death we do not do an IDG note and are actually not assigned an IDG note in HCHB.  Any time the team thinks there may be bereavement concerns before death, if I make contact then I will document what I have done, call/visit etc. in a Bereavement Coordinator IDG note and keep that updating every two weeks until bereavement ends 13 mos after death.  I use the Bereavement Contact note to document in.

    Hi Nicole and Lori - we don't use HCHB, we use EPIC, but we write an IDT note in the Plan of Care just like the other disciplines … noting the latest bereavement risk assessment and any involvement we've had with patient or family since the last IDT review.  

    ​I believe the IDT note in the Plan of Care is important, as auditors look to see that bereavement is involved from admission all the way through and that we are part of the review every 15 days; this is a clear way to note that.  At Time of Death we make a final IDT note with details about the death and a clear statement of transfer of care to the bereavement team.

    I agree with you that it's important to stay the whole time at IDT, if you can, to learn about the patient's journey and especially about the families.  At a previous Hospice, we had 5 teams and just one of me, so I couldn't stay all the way through IDT for all 5 of them!  Now it's two teams and two staff … much better arrangement!

    I also attend the entire IDG to capture information about patients and our future bereavement clients.  We use HCHB, and I write the notes in the notes of the future bereaved in the patient's record in Clinical Input. Under demographics are the listed contacts, I choose the person(s) that are identified as the future bereaved and write the notes in their record in the notes sections.  That way it flows over after the patient's death with the bereaved to their new bereavement file.  I find this very quick and efficient, and gives us a snapshot of the situation when we make bereavement calls. I hope this helps.
