  • 1.  Bereavement Measurable Goals

    How does your organization write individualized and measurable bereavement goals? We struggle with the bereaved clients who are "coping appropriately" and want minimal support services for the 13 months. We also struggle with the bereaved clients who we can't get in contact with more than once or twice in the 13 months. 
    Any insight would be appreciated.

    Hello Cassondra, 
          I'm
    not clear as to why you struggle with the fact that (my guess is your program is like ours and it's the majority of the bereaved) so many people are "coping appropriately and want minimum support." That's 'normal' and fits with the research that confirms ~ 82-85% of bereaved individuals, despite experiencing a significant loss are fairly resilient and manage to cope with the loss without any professional intervention. Although I took issue with George Bonnano's book titled The Other Side of Sadness: What the New Science of Bereavement Tells  Us About Life After Loss because I struggled with how he portrayed bereavement counseling, his research at Columbia University focuses on how resilient the majority of bereaved individuals are (check out my review of his book on Amazon :-) )
          This is also why most hospice bereavement programs focus on providing education and support as opposed to therapy - because that's all the majority of people need. That's why you might only have contact with them once or twice over the course of a year. They know where you are if they want or need more. Our goals with those individuals are to access our bereavement support as needed and to receive our newsletter over the next 16 months. With those who do seek us out and/or accept our offer to provide a 1:1 visit, we individualize our goals and determine the frequency of contacts based on what they are struggling with.  Does that make sense?

    Patti makes excellent points in her response. Our programs and goals are very similar. For those minimally involved, it is to access support and receive our mailings, and for those more involved, we tailor the goals and interventions based on their needs/strengths/interests, etc.

    I will echo those pieces said already from Patti and Diane, as well as will add that often we capture individuation in the pre and post death assessments, and IDG discussions as well. Due to the pieces Patti notes so well in her response, while a large number of folks are not interested in services, what we are able to capture in the chart in the assessments and early planning does speak to the work we are doing to prepare for the creation of more individualized/specific goals and interventions should a person wish to access services and supports. When we have been surveyed the variety and personalization seen in these assessments have been validated and affirmed as valuable and reflective of our efforts and readiness to do more.

     

    For those that do accept support, there is a great deal of variety that then unfolds in care plans and goals therein, which gets right to what you were asking about.  Our entire program provided intensive training sessions throughout the last year around care planning and we have seen all of our staff, including our bereavement team, greatly benefit from that. The problem statements, goal statements, manner of capturing patient/bereaved progression toward goals, and well written plans for next contacts has come so incredibly far!  Our survey a month ago was deficiency free and on their exit they stated that our care plans were the best they had seen in the state. It was well worth the effort we put into those trainings to come to where we are now.

    Can you share more about what  your care plan looks like and what you have done with the staff to help the goals, etc come so far? It sounds like you are doing some great training that could be beneficial on a greater scale.
