    We enclose a letter which includes the message of gratitude for being part of their loved ones care, a formal goodbye, a reminder that they can always call, and with enclosures of a poem and a bereavement satisfaction survey.

    Our care plan includes a simple phone call at 13 months, during which we explain that services are ending and that they're welcome to call if they need support in the future. ​

    We have a 13 month newsletter and inserted with it are:
    1) "end of services" letter which ​lets them know that the services are ending but we are still available to assist. Also in that letter I mention the SHP survey they would be receiving in the coming weeks (in hopes to increase our return rates)
    2) An insert for our ongoing open Grief Support Group
    3) we are considering including something for volunteer recruitment but have not fleshed that out completely yet.

    Our program is like Joel's with the 13 mo closure letter, a heads up on the EGSS survey that's coming, info on our ongoing support groups, and we also include a mention about our volunteer program and a brochure about volunteering.

    ​We do similarly what others here do. We send an anniversary support letter- letting them know we are still available and asking them to call our volunteer manager if interested in volunteering. We include a list of support groups and community resources. We also include a Bereavement Services Questioniare (with stamped envelope) asking about our bereavement services.
