Looking for some feedback on how many mailings your bereavement dept. sends to each Primary Bereaved.
I send the condolence card, 2 week packet then the 3 month, 6 month (with a self evaluation form), 9 month and 11 month.
Does anyone send a holiday card?
    Thanks

Post death, we send a birthday card one week before the patient's birthday.  We also send a one year anniversary card one week before the date of death.  The begining of November we send out a brochure we created in-house called "Life After Loss ~ Holiday's and Special Occasions".

We send out monthly mailings. I know that some people aren't a fan of this as it costs more money. However, I think that it's ideal to maintain constant contact with the bereaved and to assure them of your support and services.

Like Anne's Hospice, we send out letters monthly; plus, an additional letter right before Thanksgiving that specifically addresses grief and the holiday season.

I have received much positive feedback from bereaved as to how much they appreciate the more constant contact and how timely the messages are to what they are experiencing in their grief journeys.  I also find that monthly mailings makes them feel more connected to us which results in it being it easier for them to reach out for additional support when it is needed.

In addition to the condolence cards and phone calls, mailings, and bereavement risk assessments, our hospice also sends a single white rose to the funeral home for the patient's funeral. We have received a lot of positive feedback from the families.

We send out the condolence card, the initial grief packet, a 6th month mailing, a year anniversary letter, a holiday CareNotes packet, a discharge letter and survey, six newsletters per year, and invitations to our (3 yearly) events. We also make phone calls/leave voicemail messages at 2, 5, 9, 11, and 13 months. The feedback is primarily positive regarding the mailings; people prefer those to the phone calls. I love the idea of sending a birthday card!

We do the condolence card within two weeks, a one month mailing that lays out the who bereavement program and some grief information (the My Friend, I Care book by Barbara Karnes). We sent cards at 3, 6, and 12 months - they are nice, but don't contain any information - I am looking at changing/adding to this. We sent monthly newsletters and a final letter with a Bereavement Survey at 13 months.

The amount of calls we make depends on the need of the bereavement and their risk level.

We send a Condolence Card, followed by monthly mailings for 13 months. At the five month mark, they are sent an evaluation, which includes an "opt-out" of the mailings and/or phone calls. There is a final letter inviting them to give feedback.

In addition to monthly mailings, I sent a six month letter and enclosure, letters about upcoming events (grief support group, remembrance service, etc) and cards remembering the deceased's birthdays, wedding anniversary and DOD.
