  • 1.  Bereavement Brochures and Admissions Packets

    Hello fellow Bereavement Counselors and Coordinators --

    We're in the process of putting together a brochure or simply an information sheet that will be included in our admissions packets. 

    Do you do this at your hospice? If so, please let me know what your brochure/information sheet includes.

    I have attached the bereavement brochure we add to our admissions binders. We use this same brochure for many events and whatnot but also in the admission packet.
    We recently started putting the "Death" or "Afterward" packet in the admissions binder. The reason for this is sometimes families don't come back to the facility, home, hospital, or for whatever reason the family isn't present at the time of death, all staff know they can reference the white envelope in the back of the binder that has a letter from the bereavement coordinator and a packet for them to read on their own time about grief.

    We also have a bereavement brochure with information on what bereavement services we offer, our contact info, some information on how grief may impact
    people (little known facts, so to speak), and links to some to other resources.

    Thank you for your responses.  We offer similar information to Dawn and Veronica, we include an easy-to-read packet in the admissions folder.  It is a 4-page handout style with a lavender cover sheet titled "Bereavement Support". Inside are 1) Information letter including condolence and my contact information, 2) Bereavement Support "Afterwards.....Helping Yourself Heal" which outlines our services and early guidance, and 3) Grief Fact Sheet.

    Dawn, yours is nicely compact for a grab and go resource. That may be a future project for us.
