  • 1.  Bereavement support groups- sign-in sheets

    ​There was some discussion recently about the sign-in process for bereavement support groups.  We offer informal grief/share groups which are open to the community.  I'm looking to "update" our sign-in sheet  and process.  Is anyone willing to share the sign-in sheet and/or process that you use for your open bereavement support groups, which allows you to collect the needed information yet provide confidentiality to those signing in for group?  Thank you in advance!

    ​When HIPAA became so important several years ago we did away with the one page sign in sheet for groups and went to 1/2 sheets so each person fills out their own. We provide the NPP at the sign in table with extra copies they could take if they wanted (but no one EVER does) so when they sign the form they are also signing that they have had the opportunity to review the NPP. The title of the sign in sheet is Information and Consent for Participation and we gather info such as whether the deceased died in our care (because we too allow community bereaved to attend0, the name, date of death, their contact info, whether they want to receive our newsletter, etc. Then we scan the signed sheets so they are part of the EMR
