Hi Howard,
We receive notification about children in a family whose loved one has died on hospice care, before and after the death. In both the pre-bereavement assessment that the social worker completes at the start of care and the post death bereavement assessment, one of the questions is about children in the family. At the team meeting held weekly, we discuss the deaths and ask about children at that time as well.
We have also created a booklet about children and anticipatory loss that social workers can bring to their visit when they know there are children, it is also available on our website if families wish to have electronically.
Grief Support - Youth - Hospice of Santa Cruz County We offer bereavement services in the form of counseling sessions at our offices and at a child's school as well.
I appreciate the opportunity to have our bereavement staff meet with children prior to the death, our social worker and chaplains do a great job with this as well, at school as so often the team visits happen during the time they are at school and they don't have the benefit of witnessing the care in action.
