We use the EGSS (Evaluation of Grief Services Survey) developed by NHPCO because it is research based and we are able to compare our results with other hospices throughout the country. Rater of return is down a bit - was 12.4% the last time but had been closer to 16% earlier. Since we follow people for 16 months it doesn't go out until the 15th month after the date of death

We currently use Health Care First bereavement surveys to evaluate our program but our response rate is lower than national. I'm looking for ideas to increase those response rates. Please share your methods for increasing your response rates on Bereavement surveys?
