    Yes I would be interested too.  We have a general catch all for low risk and we do different things for mod and high, like individual counseling and recommend support groups that address the need.

    Yes, although just since earlier this year. Prior to that our Plan of Care just indicated the call cycle frequency. In preparation for upcoming state and joint commission surveys​, our agency brought in a "mock" surveyor to interview our various disciplines to make certain we're using best practices. It was determined that our bereavement plan of cares were missing an outcome or goal component. We've created a general list of plan/goal outcomes, although we're still a bit confused by the advise given by the interviewer to make our goals verbs or "measureable," since normal grief and healing are nearly impossible to quantify. Here are a few examples that we're using:

    • Plan/goal: continue monthly outreach; to help client cope with her husband's death and to achieve individual stability
    • Plan/goal: make second initial call next week; in attempt to provide grief education and ongoing support to client
    • Plan/goal: continue bi-weekly calls; to help client identify his strengths and coping skills
