  • 1.  Informed Consent

    Hello,

    A couple of years ago there was a helpful thread here that spoke about what agencies use when it comes to informed consent and scope of services. Our department is in process of looking over what we use for this and my concerns stem from using a general consent form approved by our hospital (which speaks to medical treatment, photographs, in home care, and personal valuables on our campus) and does not seem to speak to our specific work and how it is provided. Is anyone willing to share feedback or share what they use (via PDF or the like)?

    Hi Monica;

    Many folks are not allowed to share forms and that may be why you are not seeing anything. We added a sentence to a sign-in sheets for groups that indicated their signature is consent to the group.  Here's the sentence:

    By signing the grief support group attendance sheet, you are indicating consent to service and acknowledgement that the Notice of Privacy Practice is available.  For individual community clients, we created a consent for treatment form that was quite extensive.

    If others can share what they include (not necessarily the form), it might be helpful to Monica and the group.

    The topic of informed consent is always a good one:). We decided years go that during the consent process for patient admission we add that the caregviers will receive bereavement for 13 months after the death as part of the service- so we do not require additional consent for the caregiver after death. The only time we do is if we have a minor that we are caring for- so that includes any minors we might care for in the school setting.
