  • 1.  Complicated grief assessment?

    Hi, I am looking for a complicated grief assessment tool for use at admission of the hospice pt and family members.  It would be really ideal if the form was electronic or based in AllScripts.  Realistically I would be happy with a paper form.  Can you help me find something?  Thanks I appreciate it.

    Hi Bernie,

    All scripts has a good pre-death berv risk assessment that our social workers complete during their initial social work assessment visit.  It covers risks, coping, support, the nature of the relationship, etc. and you can assess more than one family member in that section of their assessment as well.
