We are seeking input on the Family Evaluation of Bereavement Services (FEBS). Specifically, we are seeking feedback from current, past, and non-users of the NHPCO FEBS survey and report through a brief survey. The information gained from the survey will be used by NHPCO to make revisions and additions to the content and format of the FEBS survey and report. You may have already received an email invitation to complete the survey. Thank you to those who have already completed the survey!

The deadline for completing the survey is December 21, 2015.
