  • 1.  Bereavement representation in IDG

    I am wondering what other hospice programs do in terms of having bereavement representation at IDG. We were complimented during our survey regarding the presence of bereavement coordinators at all teams, however we are unable to determine if this is a requirement or not. I would really appreciate a look into how other bereavement coordinators interface with teams.

    Our role has been to:  read the deaths and provide a brief ritual to assist the staff with processing the hard work;  maintaining presence in the room to educate the team on anticipatory grief and loss issues that are discussed and be aware of any potential high risk bereavement issues on our teams.

    LuAnn,

    We have a grief counselor attend all IDG meetings. Their primary purpose is to provide consultation on existing patients (if needed), report on any bereavement involvement with current patients or families of current patients, and to receive information from the team regarding needs, risks, and concerns of family members of recently deceased patients for the purpose of bereavement follow up.

    Within  our MHC Hospice programs we also do as Luann mentioned re: Bereavemetn Coord role  at IDT attendance and support. It has been our practise for many years and an important aspect of providing  pre and post  death care to hospice families and  to function as a team member.

    Hi - We have the bereavement coordinator attend IDG meetings. Like Daniel, they provide consultation and report on involvement with current families, and they learn information about risks and concerns for recently deceased patients. They usually start the meetings with deaths.  The patients name is read, info is shared, and then some teams place a stone in bowl/vase for each deceased patient.  In addition, as patients are being discussed, the bereavement coordinator checks the EMR and checks if the initial assessment for bereavement risk factors was completed and if appropriate, a plan of care was made.  They also check the family and friends tab to see if the address and phone number of the caregiver/bereaved was entered.  If these are incomplete, they let the SW or Team Leader know.  It's really helped!

    We do as you and Daniel do – because we have JCAHO certification it's a requirement that a 'counselor' (meaning spiritual care or bereavement) remain for the entire time the team meets so we collaborate closely with the  chaplains to make sure one if not both are always there
