Our return rate on the 13 month bereavement survey was very low this last quarter, 7%.  I am wondering if other hospice bereavement teams are using the NHPCO bereavement survey and if so, what are your return rates?  If they are good, are you mailing them out with a letter, calling before the mailing?  Any tips would be appreciated.

Are other bereavement teams using a different survey tool?  If so, what tool are you using or did you formulate your own follow up survey.  Are you having success in return rates on those?

    Thanks so much,

We developed an in-house survey based on the services and programs we offer, and it provides opportunity for both quantitative and qualitative data collection. We have a volunteer call and administer the survey over the phone (it isn't a very long survey; only 8 questions). Our response rate for this calendar year to date is 45%, which is up from 34% last year.

    Hi -

Our response rate is also less than average but not as low as 7%.  It has been as low as 12% and as high as 18%.  We are considering developing a new 13 month survey which will be considerably shorter.  We are piloting a concurrent survey (6 months) and are hoping for a good return rate and allow us to respond before the 13 months is up.

One suggestion that came from another hospice is to send the survey on colored paper. We do send a different color every month.  It did not help in our case, but it may for you. Good luck.
