  • 1.  Is there a Medicare or NHPCO requirement for when an assessment and care plan must be completed by?

    Hello,

    I was wondering if there is a Medicare or NHPCO requirement regarding the time frame a Bereavement coordiantor has to complete a bereaved assessment and care plan? I was told 72 hrs, but often our Bereavement Coordinators do not even get a hold of bereaved by then. Our Hospice was thinking a guideline of attempting to have it complete within 2 weeks (assuming you reached bereaved) would work.

    The medicare regulations are loose - they require that hospices have an organized program for bereavement.
    Our state regulations are much more stringent. I live in Missouri, and I believe we are supposed to make contact within 72 hours. That is what our organization does, and while I haven't looked at the regulations lately I know what we do is based on state regulations.

    Just recently my administrator, Frank Houston sent me a copy of the Missouri Regulations concerning Bereavement Services from List Serve.  This is from the Bureau of Home Care - - the Surveyors  in Missouri:
            State regulations at 19 CSR 30-35.010 (2) (G) (5) (b) says: "Within two months following the patient's 
            death, there shall be an assessment of risk of the bereaved individual and a plan of care that extends
            for one year appropriate to the level of risk assessed".

    ​Hi -
    Are you referring to pre-death or post death?  Patients/families should be assessed (and seen) for bereavement risk factors upon admission and throughout the course of care.  There are no COP on when an assessment should be completed post death, but this would be based on any pre-death risk factors and then what happened at the time of death. Based on this information, we enter a Plan of Care at the time of death.  We do not do an actual bereavement assessment unless we are able to meet with the bereaved over the phone or in-person.

    Our hospice has a census of close to 150 patients and we have two full-time counselors who establish plans of care. The social workers enter an initial bereavement assessment when they meet with the patient/family the first time. We add a post-death assessment if the patient dies prior to the social worker assessment, because unfortunately, that's the case many times. We enter it when we prepare for our IDT meetings.

    Our hospice has a census of 250 and a home based palliative care census of 200.  Our bereavement program serves both, but there are very few deaths in palliative care.  We have three part time counselors that work 20, 20 and 28 hours a week. We also have one coordinator who works 36 hours a week and their role coordinates care plans, mailings, events, phone calls, organizing our groups (rooms, flyers, materials), maintains our printed materials, etc.
