Annual Memorial Service – Sponsored by the National Council of Hospice and Palliative Professionals (NCHPP)


NCHPP’s Annual Memorial Service will be held at NHPCO’s Clinical Team Conference on Monday, September 13, 2010 at 4:00 pm. This special gathering of hospice and palliative care professionals pays tribute to colleagues (staff and volunteers) who have died within the past year. If you will be attending The Clinical Team Conference, please plan to attend. If you cannot attend but wish to submit the name(s) of colleagues who have died within the past year, their names will be read aloud during the service and entered into NCHPP’s Annual Memorial Service book, providing permanent recognition. Through readings, music, poetry and remembrance, NCHPP is proud to honor and recognize hospice and palliative professionals and volunteers whose lives have made a difference and whose memory is cherished.

To submit a name, submit the following to Helen Ennis at hennis@nhpco.org by Wednesday, September 8:
• Person’s full name (with pronunciation tips for unusual or unfamiliar names)
• Person’s role within your organization (i.e. volunteer, nurse, etc.)
• Full name of your organization
• City and state where your organization is located (or site where this person worked/volunteered)
